Forms (needed to attend tryouts)

Your athlete may not participate at tryouts if they do not have this paperwork filled out. Attending tryouts is mandatory in making a team.

If unable to access a printer, these forms will be available to fill out at tryouts but please get there early to be able to do that!

Teams/ Tryouts

Parent Meeting: Wednesday, November 5th

6-6:45pm @ MHS commons

TRYOUT DATE: Saturday, November 15th, 2025

12s and 14s tryout time: 11am-1pm

16s and 18s tryout time: 1pm-3pm

TRYOUT LOCATION: Jefferson County Middle School

Enter through the westside doors where the buses drop students off.

There will be one team per age group. Teams will be made up of 8-12 players. 16s and 18s will depend more on skill rather than age.
